CAMP SCHEDULE
Ages 5-11 . . . 9am-12pm
Ages 12-17 . . 1pm-4pm

DROP OFF AND PICK UP TIMES: Drop off time for ages 5-11 is between 8:30-9:00am (camp starts promptly at 9:00am); pickup time is 12:00pm. Drop off time for ages 12-17 is between 12:30-1:00pm (camp starts promptly at 1:00pm); pickup time is 4:00pm.

REFUND POLICY
No refund will be given for no-shows unless a 24-hour notice has been given before the first day of camp. Please send this notice to ant@anthonyatkinsonjr.com. No refund will be given for homesickness, lack of participation (not participating), or campers sent home for disciplinary reasons. If it is a medical emergency, please email a doctor’s note to ant@anthonyatkinsonjr.com to receive a full refund. If we are forced to cancel camp, a full refund will be given.

All participants in sponsored activities by Not Good But Great Basketball Camps must agree to and abide by the following:

• If an athlete or coach is sick, they should not come to camp.

• Anyone showing symptoms or registering a temperature greater than 100.4 will not be permitted to enter and will be sent home.

• Agree that there is an inherent risk of exposure to COVID-19 in any public place where people are present and that COVID-19 is an extremely contagious disease that can result in severe illness or death.

• Athletes may arrive no more than 30 minutes before camp starts and must be picked up/leave no more than 30 minutes after camp concludes.

• All players and coaches must wash their hands prior to entering the gym area and after leaving the court.

• Athletes and coaches should bring their own water. Water fountains and bottle stations will not be open.

• Hand sanitizer will be available; however, athletes and coaches can bring their own.

• By attending Not Good But Great Basketball Camps, you are voluntarily assuming all risks related to exposure of COVID-19.

• Agree to follow the procedures set forth within Daniels Chapel. Not Good But Great Basketball Camp Staff have put in place numerous preventative measures and enhanced cleaning protocols to reduce the likelihood of spreading COVID-19 in Daniels Chapel. However, they cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending Not Good But Great Basketball Camps could increase your risk and your child(ren)’s risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily agree to the participation terms described above and assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the camp and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19, at the camp may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Camp Directors, Coaches, Athletes, and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at the Camp or participation in Camp programming (“Claims”). On my behalf, and on behalf of my child(ren), I hereby release, covenant not to sue, discharge, and hold harmless Daniels Chapel and Not Good But Great Basketball Camp, its employees, agents, representatives, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expense of any kind arising out of or relating thereto. I have read this release and waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and have signed it freely and voluntarily without any inducement, assurance or guarantee being made to me and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Daniels Chapel and Not Good But Great Basketball camps, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any camp activity.

PARENT STATEMENT

I hereby state that my child is in good mental and physical health condition to participate in the activities provided by including but not limited to all aspects of the basketball camp. I am fully aware that any activity involving motion, height or athletic activity creates the possibility of serious injury. I hereby release “Not Good But Great Basketball Camp” staff and Daniels Chapel from liability to the above named athlete, of the person claiming through him/her, arising from injury to the person or property of the above named athlete including any event sponsored or sanctioned by “Not Good But Great Basketball Camp”and Daniels Chapel, and or travel to and from such activities.

I understand that “Not Good But Great Basketball Camp”, has the right to deny admittance to any student not meeting the standards of the program as it sees fit. I also agree not to hold these parties responsible in the event that my son/daughter/child engages in inappropriate conduct (including, but not limited to disruptive or volatile behavior in or out of camp, etc.) or becomes involved in any activity or with any persons not associated with “Not Good But Great Basketball Camp”, or its scheduled program and that “Not Good But Great Basketball Camp”, has the right to send him/her home for inappropriate conduct. I further attest that the information contained in this application is correct to the best of my knowledge. In addition, I have agreed to the policy and fee statement and agree to comply.

I hereby give “Not Good But Great Basketball Camp”, and Daniels Chapel staff permission to photograph and/or videotape the student for educational or promotional purposes.
